The practice participates in most major insurance plans. If our practice is not in your provider book, please ask us as many of these books are outdated. If your insurance plan requires a referral from your primary care physician, it is your responsibility to arrange this prior to your visit and to bring the written referral with you to your appointment. The referral may need to include special tests, and patients should call the staff ahead of time so that this information can be included on the referral. If you have an HMO that requires a written referral and you do not bring a valid referral, you may be asked to reschedule your appointment until a referral is obtained. Please understand this is your HMO's policy and complaints should be taken up with the HMO.
*************** We do NOT participate with Tricare Standard, Tricare Prime, or PHCS ***************
If Dr. Rosenberg orders any testing to be done at any facility other than our office, you should contact your insurance company to see if any referrals or authorizations are needed. This should be done prior to the appointment. Our office is happy to call insurance companies to get authorizations, however, if a referral is needed, you must go through your primary care physician. Please allow as much time as possible, as some insurance companies require a "review process" which can take several days. It is also recommended that you call the facility to see if they have the appropriate authorization and/or referral ahead of time. These facilities may have to reschedule your appointment if they are lacking a referral or authorization. Please contact Katie or Trish if you need help with an authorization or if you have any questions about the process.
